Knowing when to slot a Heart and when to hold back can make the difference between obliterating bosses and stumbling in critical phases.
When to Use a Malignant Heart
Use Malignant Hearts when they complement your core build and playstyle by enhancing damage, survivability, or utility in ways your gear and passives cannot. If a Heart’s affix synergizes with your class mechanics or skill rotations—for example, a Vicious Heart boosting critical hits for a crit-focused Rogue or a Brutal Heart providing clutch healing for a tanky Barbarian—they multiply your effectiveness dramatically. Hearts also shine when tackling high-end content like Tormented bosses, Vault leaderboards, or complicated Pit encounters, where every percentage point of gain counts.
Another prime moment is in group play, where Hearts that boost crowd control, resource returns, or team buffs sustain synergy and speed clears.
