Craft Tier 50+ sigils at the Alchemist, enter solo or with a group, and deposit 12 Distilled Fear at the Frozen Altar for the boss summon. Burst the Beast down in 4-6 minutes, loot the hoard for 1-3 dolls guaranteed in Torment, then portal out and repeat. Cycle this for 45 minutes straight, banking 20+ dolls, then pivot to restock Fear via a single Tier 35 Nightmare Dungeon.
