These hearts drop from special Malignant enemies—mini-bosses and elites marked by orange burrows in dungeons, Helltides, and world events. Spot a pulsing Malignant Heart on the ground after defeating one; interact to spawn a tougher, empowered version of that foe. Victory rewards a Caged Heart matching the burrow's color: red Vicious for offense, blue Brutal for defense, purple Devious for utility, or rare multicolored Wrathful supers. Socket them into jewelry like amulets or rings via the Occultist, replacing standard gems for build-defining effects that scale with world tier.
Hearts unlock progressively: basic ones at World Tier 1, advanced at Tier 3, and Wrathful at Tier 4. Vicious Hearts boost raw damage, like Tempting Fate granting 40-60% Critical Strike multiplier at the cost of 15-20% non-crit penalty, or Focused Rage for Barbarians spiking crit chance after Fury dumps. Brutal options fortify survival, such as Prudent Heart's 2-4 second Immunity after big hits (110-second cooldown) or Revenge suppressing 10-20% incoming damage before exploding it as fire on defensive skill use.
